Life Impact Healthcare Services LLC provides person-centered home and community-based services for eligible individuals enrolled in Virginia Medicaid waiver programs. Our team works closely with individuals, families, Support Coordinators, Case Managers, and healthcare professionals to develop services based on each person’s unique needs, preferences, and authorized Plan of Care.
CCC Plus Waiver
The Commonwealth Coordinated Care Plus (CCC Plus) Waiver supports eligible older adults and individuals with disabilities who require assistance to remain safely in their homes and communities.
Life Impact services may include:

Life Impact Healthcare Services LLC also supports individuals with intellectual and developmental disabilities through applicable Virginia DD Waiver services.
Virginia’s DD Waivers include the Community Living (CL) Waiver, Family and Individual Supports (FIS) Waiver, and Building Independence (BI) Waiver.

Who We Serve
We support seniors, adults and children with disabilities, individuals with developmental disabilities, and individuals with complex healthcare or personal-care needs who qualify for applicable Virginia Medicaid services.
